Vital Statistics
To obtain certified copies of vital statistic records, please send the following to
Borough of Ship Bottom, Attention: Registrar, 1621 Long Beach Blvd., Ship Bottom, NJ 08008
-Vital Records Request Form
-Check or money order made payable to the Borough of Ship Bottom
-Stamped, self addressed envelope
-Proof of identification (copy of a drivers license)
Charge for copies is $15.00 for the first copy and $5.00 for each additional copy.
Marriage License/Civil Union Application
-Must bring Birth Certificate-Drivers License- Social Security Card-Divorce/Death papers & Witness to apply
-There is a waiting period of 72 hours after applying for the marriage license
-$28.00 application fee in the form of cash, check or money order made out to the Borough of Ship Bottom
