  To obtain certified copies of vital statistic records, please send the following to:

    -Vital Records Request Form

    -Check or money order made payable to the Borough of Ship Bottom

    -Stamped, self addressed envelope

    -Proof of identification (copy of a drivers license)

Charge for copies is $15.00 for the first copy and $5.00 for each additional copy. 

Marriage License/Civil Union Application

  MUST BRING WITH YOU WHEN YOU APPLY:

    -Birth Certificates

    -Drivers Licenses

    -Social Security Cards

    -Divorce/Death papers

    -a Witness who knows both applicants

    -$28.00 application fee in the form of cash, check or money order made out to:

•   There is a waiting period of 72 hours after applying for the marriage license.
